Is this meant to replace my regular doctor?
No. Diverse Health virtual primary care is designed to work alongside your traditional healthcare. We help you stay connected to care between office visits — managing chronic conditions, refilling medications, coordinating referrals, and addressing concerns faster. For comprehensive physicals, imaging, and procedures, we recommend continuing to see your in-person provider.
What can your providers help with?
Our FNPs manage hypertension, diabetes, cholesterol, weight management, medication refills, preventive care, lab follow-ups, and referral coordination. We focus on keeping you healthy between in-person visits and making sure nothing falls through the cracks.
How is this different from urgent care apps?
Urgent care apps focus on one-time visits with rotating providers. We build ongoing relationships — the same provider who knows your history, manages your medications, tracks your progress, and coordinates your care over time.
What if I need a specialist?
Your primary care provider can refer you to specialists in your area. We coordinate with your insurance and help facilitate the referral process.
How is this different from the mental health service?
Our psychiatric service is a comprehensive virtual specialty practice — psychiatric evaluations, diagnosis, and ongoing mental health treatment. Our primary care service is a healthcare support and care coordination model focused on chronic disease management, medication refills, and preventive care. Both are available on the same platform but serve different clinical needs.
